Alwyndor is looking for dedicated and caring TCP and CAP Coordinator / Enrolled Nurse's to join our team.

Alwyndor’s welcoming, high quality facilities include 144 residential, dementia, respite care beds, transitional and care awaiting placement beds, together with allied health, therapy and wellness programs and home support packages that support people in Adelaide’s southern metro communities.
Responsibilities include but are not limited to:
Liaise with stakeholders to discuss care needs and identify goals- Collaborate with multi-disciplinary teams to ensure a person centred focus is maintained in all aspects of the consumers life, and care needs are met through a planned and coordinated approach
- Undertake comprehensive contemporary, systematic and accurate nursing assessment of new clients to formulate a plan of care
- Maintaining a physical and psychosocial environment which promotes safety, security and optimal health for our residents
- Assisting in formulating and implementing a plan of care in consultation with residents/representatives
- Maintain accurate and confidential medical records of residents
- Ensure the provision of a high standard of care for the residents based on the nursing process and standards for residential aged care
- Issue medications prescribed by the doctor in accordance with duty of care and standards of nursing practice
Essential:
- You must be an Enrolled Nurse with current AHPRA registration
- Completed degree or Diploma of Enrolled Nursing
- Demonstrated experience conducting Clinical assessment reviews and the relevant documentation with clients and families
- Demonstrated experience conducting individual person-centred care plans
- Commitment to maintaining confidentiality as per Alwyndor policy
- Ability to use Microsoft Suite and other industry related programs effectively
- Previous aged care experience is desirable.
- Comply with all mandatory vaccinations required by the government to work within the Aged Care sector including Influenza and Covid-19 vaccinations.
