Team Leader - Home Care Packages SEM
We are looking for an experienced and enthusiastic Team Leader to join our Home and Community Care Services team. The successful applicant will be responsible for the day-to-day supervision of the Home and Community Services delivered from the nominated site including the deployment and implementation of systems and processes in line with mecwacare policies and procedures, and contractual and legislative requirements. This role leads a team of Client Service Coordinators.

- Ability to work appropriately with IT applications, including the Microsoft Office suite and client database system.
- Excellent communication skills both written and verbal.
- Demonstrated experience in a similar role.
- Knowledge and experience in the Community Care sector.
- Experience with continuous improvement and risk management systems and processes.
- Knowledge of CHSP, HACC guidelines, CareLink + and My Aged Care Portal will be highly regarded.
