Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Holroyd and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region. The primary purpose of the Team Leader of Clinical Treatment and Care Coordination Team in WSLHD adult community mental health services  is to provide person centred recovery oriented mental health service incorporating whole person approach and life course approach to people with chronic persistent mental illness and complex psychosocial needs living in Western Sydney LHD area. As the leader of a multidisciplinary team this role is required to ensure professional clinical and coordinating services that include mental health triage, comprehensive mental health assessment, treatment, ongoing care co-ordination and discharge planning services to individuals. This role also needs to collaborate with other services such as police, ambulance, government and nongovernment agencies, etc. This role must ensure that the team meets all its KPIs of WSLHD-MHS and NSW Health.
