The Transition Officer key responsibilities are:
- Assist in preparing young people to participate and engage in education, training and employment programs as required; and support the development of their interpersonal and self-regulation skills.
- Work collaboratively as part of a team to plan, deliver and refer young people to education, training, and employment.
- Work effectively within a statutory and non-statutory environment to provide support to young people during education, vocational training, work experience and employment.
- Collect and maintain accurate data and client records in accordance with relevant statutory, legislative, policy and procedural requirements.
- Identify, liaise, and maintain strong working relationships with relevant internal and external stakeholders and community partners.
- Contribute to a departmental culture that ensures sensitive and effective communication with people from Aboriginal and/or Torres Strait Islander
backgrounds as well as an appreciation of cultural protocols and an awareness of the impacts of historical policies and practices.
