We are actively seeking a Transition Pathways Officer for a temporary full-time position until 30 June 2025, unless otherwise determined. The position will be based at the Spinifex State College campus and provide a broad range of services to maximise transition outcomes for students at-risk of not successfully transitioning from post-compulsory schooling before the completion of Year 12.

About the Role:
As a Transition Pathways Officer, you will lead case management support to identify suitable pathway options for young people. Further, you will develop and maintain effective school-based and stakeholder networks, and collaborate with stakeholders to achieve positive outcomes. A candidate with strong networking and communication skills, great time management and the ability to deliver positive outcomes whilst working autonomously and within a wider team, is greatly desired.
Responsibilities include, but not limited to;
- Provide appropriate support and intervention to young people and their families to assist in the transition of young people into a viable education, training or employment option.
- Facilitate the development of a school to post school transition plan for young people who leave school before the completion of Year 12 and provide appropriate case management and support as required.
- Forge links with providers of employment services, community agencies, training organisations and employers to create opportunities for young people to engage in programs that support successful transitions from school into employment or training.
- Establish and maintain effective networks with school based support staff across a cluster of schools to maximise the transition outcomes of students at risk of not successfully transitioning from school before the completion of Year 12.
