Programmed Care provides care, support, and home services to clients throughout Australia across a variety of programs. Our Care Consultants are responsible for ensuring our clients are provided with wide range of support services by high quality staff across the community. We provide care and support services to the Disability, Veterans Health, Aged Care and Community Nursing industries.
The role:
Programmed Care is the community and in home direct care division of Programmed Health Professionals. We are seeking an experienced Registered Nurse, Enrolled Nurse, or Occupational Therapist to fill the role of Transitional Care Package Coordinator.
You will liaise directly with the Transitional Care Package discharge team in establishing care at home for recipients, basing your assessment on the guidelines set out by the TCP Program.
You will be responsible for the initial establishment and/or review of the community nursing or home care plan following a comprehensive standardised assessment. The role includes coordination of direct clients’ care, highlighting the need for referral to multidisciplinary or specialty services.
Responsibilities:
- Accurate assessment and review of client care including multidisciplinary referrals.
- Coordination of service requirements in relation to a total Care Package.
- Enhance relationships between Programmed Care and stakeholders.
- Clarification of funding parameters prior to setting up visit schedule.
- Working within TCP guidelines in conjunction with Programmed Care procedures.
About You:
- Focus on delivering engaging quality healthcare and coordination of best practice clinical care that supports and enhances clients’ needs and wellbeing.
- Excellent communication and collaborative skills
- Current AHPRA Registration
- Demonstrated understanding of the relevant Laws, Acts, Regulations, Charters, and Codes of Practice
- Knowledge and relevant experience in clinical services in a complex nursing and/or aged care setting (experience in community setting will be favorably looked upon)
- National Police Check (within the last 12 months)
- Current Working with Children Check
