- The Wellbeing and Health in-reach Nurse (WHIN) Coordinator CNS2 is responsible for facilitating identification of health, wellbeing and social needs and the coordination of appropriate early intervention, assessments, and referral to services for school-aged children and young people attending designated primary and secondary schools.
- The WHIN Coordinator CNS2 is a Registered Nurse who works in identified local schools and applies their specialist clinical nursing knowledge, experience and skill in providing complex health care coordination for school aged children, young people and their families in these schools.
- The WHIN Coordinator CNS2 role requires leadership, specialist skills, capacity for a significant level of autonomy and independent work in an interagency context. Coordination of access to services across multiple agencies, professions and groups will be part of the role.
