As a Youth Caseworker in the After Care program, you will provide crucial casework and support to young people experiencing a range of issues, who are exiting the out of home care system and transitioning into adulthood.

- Manage a caseload of young people aged 17-24, facilitate plan goals, provide daily support, supervision, and develop personalized goals and aspirations.
- Develop and deliver services and supports to help participants transition to independent living, employment and financial management.
- Ensure comprehensive and up-to-date documentation, including case notes, reports, and records, following record-keeping procedures and policies.
- Build and maintain relationships with internal and external stakeholders to support program objectives and participants' goals.
- Collect and monitor data for program evaluation and assist in preparing evaluation reports.
- Adhere to policies and procedures, especially regarding client rights, and monitor child protection, safety, and wellbeing issues.
- Adapt working hours to meet participants' needs, including availability before and after education/work hours as required.
- Engage in ongoing professional development, attend supervision sessions and team meetings, and comply with workplace health and safety policies.

- Tertiary qualifications in Community Services or related field
- Casework experience in Foster Care or Youth Services
- Demonstrated ability to produce clear and concise written material
- Understanding of the Children and Young Persons (Care and Protection) Act 1998
- NSW Driver's License and Working with Children Check
- Ability to participate in a rotating on-call roster (remunerated)
