Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory.
We work to address the social determinants of health including the broader issues that affect our people's health, through evidence-based research and public advocacy. We also work to empower individuals, families, and communities to optimise their health.
ABOUT THE ROLE
The Case Manager for our Youth Diversion Program (YDP) supports young people involved in the justice system to implement positive lifestyle changes, re-engage with the community, and reduce re-offending.
This role will support each young person to participate in developing their individually tailored support plan and look at the contributing factors as to why they have committed the offence.
WHAT YOU WILL DO
As the Case Manager within our Youth Diversion program, you will adopt an inclusive approach to meet the needs of young individuals navigating the justice system. Effective communication methods are essential for establishing and maintaining relationships with clients and colleagues from diverse linguistic and cultural backgrounds, fostering an environment of respect and trust.
You will collaborate with external agencies to cultivate optimal service delivery and referral processes, all while adhering to legislative and ethical guidelines to ensure the provision of high-quality services and protect client rights. Your administrative responsibilities will entail thorough documentation and reporting, ensuring compliance with funding agreements and legal mandates. Emphasising the creation of safe learning environments, you will address the complex needs of clients as a primary focus.
SELECTION CRITERIA
- minimum of 2 years' experience in youth sector case management, the ideal candidate possesses a deep understanding of Restorative Processes and Trauma-Informed practice.
- Experience supporting vulnerable youth, particularly with an awareness of social justice issues impacting Aboriginal and/or Torres Strait Islander communities.
- Demonstrates awareness of Aboriginal culture and history, and sensitivity in navigating issues affecting Aboriginal children, families, and individuals affected by Stolen Generations' policies, while effectively engaging and supporting young people with challenging behaviours.
- Experience within multidisciplinary team structures, in interacting with clients, carers, and professionals from diverse cultural backgrounds.
- Strong organisational skills to develop and implement comprehensive case management plans, maintain accurate case notes, and produce detailed case management reports.
- Proficiency in utilising electronic information systems ensures efficient maintenance of clinical and service delivery records, contributing to streamlined operations and effective service provision.
- Tertiary qualifications and/or relevant training in social welfare (desirable)
- 6 weeks annual leave.
- 5 days paid leave during end-of-year shut down (additional to annual leave).

- Ability and willingness to obtain a Working with Children Clearance (Ochre Card).
- Current driver's licence.
- Current CPR certificate or the preparedness to gain one within 6 months of commencement.
