About the Opportunity

As a service provider MCM’s work is focused on supporting people to take charge of their own lives and participate fully in community life. Our Healing Oriented Framework guides us in our trauma informed knowledge to enhance trauma responsiveness across all programs and deepens the capacity of teams and services in complex environments to provide healing-oriented care.

Young Women, Families & their Children (YWFC) is one of the many integrated services located at Frontyard. YWFC Case Managers provide delivery of rapid engagement, culturally safe, flexible, and holistic case management services to young people experiencing homelessness and/or who may be pregnant or parenting, to meet young people’s needs and goals. YWFC utilises creative and innovative pathways to disrupt disadvantage and create pathways out of homelessness, while also providing support in relation to pre and post-natal care, family preservation and child reunification.

About you

We are looking for someone who can:

  • Effectively manage a case load of young people through a healing-oriented framework. Utilising creative and innovative pathways to disrupt disadvantage and create pathways out of homelessness.
  • Develop advanced case planning and crisis interventions to meet the needs of young people presenting with multiple and complex needs, including risk assessments and safety plans.
  • Provide case management relating to pre and post-natal care, family preservation and child reunification.
  • Assist young people to access ongoing accommodation options, particularly supported accommodation, family reconciliation, private rental, shared housing and social or transitional housing in accordance with their case plan.
  • Ensure services are delivered within the framework of MCM’s policies and procedures, legislative, requirements, and meet the relevant service standards. All employees are required to comply with the Child Safe Standards.

Essential:
  • A qualification in youth work, community development or a related tertiary qualification or extensive experience working in the community services sector.
  • Demonstrated experience implementing child/parent focused programs and services for children, parents and their families who have complex needs such as homelessness, mental health presentation or AOD related issues, family and/or intimate partner violence, working in a case management framework.
  • Sound understanding of maternal pregnancy care pathways, family violence frameworks, key child development and the factors that can impede a young person’s development, as well as an understanding of the issues facing vulnerable parents in their ability to ensure the safety and wellbeing of their children.
  • Understanding or experience working with young people at risk or experiencing homelessness, including knowledge of healing-oriented approaches.
  • Strong passion for working with young people and ability to actively and assertively engage young people experiencing complex needs.
  • An understanding of the homelessness service system with knowledge of patterns, trends, systemic issues and principles in working with at risk young people.
  • Knowledge of the legislative requirements when working with at risk young people.
  • An understanding of the requirements for ensuring child safety.
  • Excellent communication and problem-solving skills encompassing interpersonal, verbal and written, and negotiation skills.
  • Provide First Aid certificate (formerly First Aid Level 2).
  • Strong organisational, time management and computer skills.

  • Working with Children check and NDIS Worker Screening check or ability to obtain
  • Valid Victorian Drivers licence
  • Right to Work in Australia
