About the role:
- Registered Aboriginal Health Practitioners (AHP) are responsible for delivering safe, high quality clinical services, patient care, health promotion and education as part of a multidisciplinary team
- The AHP works within the clinical team environment with the aim of improving the health status of Aboriginal and Torres Strait Islander people through the provision of comprehensive and culturally valid primary health care services
- Working closely with patients, their families and carers, this is a role where you will witness the direct positive impact you're making in the community, as part of a close-knit team
- Provide day to day health service activities within the context of the medical service's model of care in a professional, confidential and culturally safe manner
- Provide education for clients and families on the roles of various providers involved in their care
- Conduct opportunistic and scheduled 715 health assessments, immunisations and treatment of clients within own scope of practice and delegation of clinical authority
- Triage and support Aboriginal clients through clinical services and actively participate in, and/or facilitate aspects of care as appropriate
- Support clients and families with information, resources and advice in response to specific requests and/or identified need
- Identify opportunities or individual, family and community education to build knowledge about wellbeing including healthy life choice, risk factors, chronic conditions, and the breadth and availability of health care and related services
Experience & qualifications:
- A person who is reliable, having a caring and compassionate personality, are flexible and willing to work with a wide range of people
- National Police Check
- Working With Children check
- Hold current National Registration as an Aboriginal and Torres Strait Islander Health Practitioner with the Australian Health Practitioner Agency (AHPRA)
- Pre- employment medical assessment
- Hold and maintain a current Australian issued Driver’s Licence.
- Proof of covid-19 vaccination
- Is of Aboriginal decent; self identifies and is accepted as an Aboriginal person by the local community
