About Moorditj Koort Aboriginal Corporation
Moorditj Koort Aboriginal Corporation (MKAC) is a community controlled, not-for-profit organisation supporting the health and wellness of Aboriginal and Torres Strait Islander peoples living in Western Australia. The Vision is to contribute to healthy and flourishing Aboriginal communities across WA by providing access to a holistic one-stop-shop for health and wellbeing support.
The Access and Engagement Officer (AEO) will support people who are eligible to access My Aged Care (MAC) or the National Disability Insurance Scheme (NDIS). The Access and Engagement Officer (AEO) will work closely with My Aged Care (MAC), the NDIS and their Partners in the Community (PiTC) to ensure the community is well informed of the services available and support individuals, their families and carers to navigate the Access and implementation process.
Duties will include but are not limited to:
- Supporting clients to access My Aged Care (MAC), the National Disability Insurance Scheme (NDIS) and other community or mainstream services.
- Supporting Elders through the My Aged Care assessment process and work collaboratively with the Aged Care Assessment Team (ACAT) to provide culturally safe and appropriate assessments
- Supporting clients/participants through the NDIS access process and work collaboratively with the NDIS and Partners in the Community (PiTC) to provide culturally sate and appropriate services
- Act as a cultural broker between the community My Aged Care (MAC) and the National Disability Insurance Scheme (NDIS)
- Community engagement /outreach to promote My Aged Care and NDIS.
- Identify people who may have a disability or who are eligible for MAC and build relationships of trust, cultural safety and understanding.
- Advocate for clients/participants to ensure they receive the appropriate supports and services to meet their needs, goals and aspirations to live a good life
- Liaise and work collaborative with Moorditj Koort teams to support clients accessing our services and programs
The successful candidate must have the following skills.
- Knowledge and understanding of community protocols and preferences, to work in ways that meet the expectations of local communities.
- Demonstrated knowledge and understanding working within Indigenous communities with the ability to engage and work effectively with Aboriginal and Torres Strait Islander people.
- Demonstrated effective verbal and written communication and interpersonal skills including the ability to develop collaborative partnerships.
- Demonstrated experience in managing general correspondence and emails, completing forms and documentation.
- Demonstrated computer skills including Excel and ability to work with client management systems.
- Demonstrated ability to work both independently and as a team member with minimal supervision.
- Knowledge and understanding of the National Disability Insurance Scheme (NDIS)
- Knowledge and understanding of the My Aged Care (MAC)
- Understanding of and commitment to the principles of Equal Employment Opportunities, Work Health and Safety, Disability Services and Quality Improvement principles
Desirable
- Experience using the My Aged Care portal
- Experience and/or qualifications in relevant field such as, aged care, community development, social services
- Case management experience
- Strong advocacy knowledge and skills
Essential requirements
- Valid WA drivers' license
- National Police Clerance
- NDIS Worker Screening Check
- COVID-19 Vaccination Certificate
- First Aid or willing to obtain
