So, planning ahead is relevant to everyone.
Advance Care Planning (ACP) provides a clear process for talking about and documenting preferences for a person's future health care. The choices made through this process only come into operation if they no longer have capacity to make their own decisions regarding medical care and treatment. The way in which a person completes the relevant ACP documentation will guide medical decision makers, their family and presiding doctors to make decisions based on their expressed wishes and expectations.The Advanced
Care Planning Clinician works as part of a team who are responsible for providing ACP support across PH and in the community.
What You NeedWe anticipate you will have:
- Qualified Social Worker with eligibility of the AASW.
- Sound understanding of Advance Care Planning aims and underlying principles.
- Demonstrated commitment to person centred care and patient advocacy.
- Workplace experience supporting and mentoring colleagues / staff.
- Willingness to learn, maintain and advance knowledge relevant to the position.
- Commitment to work as part of a specialised team to achieve the ACP Program aims.
- Ability to supervise undergraduate students.
- Current Victorian Driver's Licence.
- Aged Care Police Check.
- Working with Children Check.
- Highly developed communication and interpersonal skills and demonstrated ability to work collaboratively within a multidisciplinary team.
