Wirraka Maya Health Services Aboriginal Corporation is an Aboriginal Community Controlled Health Service designed to ensure our clients' journey through the health system meets their medical, health, social and cultural needs with a vision that Aboriginal and Torres Strait Islander people in the Pilbara have the opportunity to live a health life, with access to culturally appropriate, holistic primary and allied healthcare; social and emotional wellbeing services and education.
As part of the Social and Emotional Wellbeing (SEWB) department, this role is crucial in providing ongoing support to individuals within the community.
Key Responsibilities: -
Delivering Aftercare Support to individuals who have completed a program or treatment for mental health or substance abuse issues.
Conducting regular check-ins with clients to monitor their progress and provide assistance as needed.
Collaborating with other healthcare professionals and community organisations to ensure a holistic approach to Aftercare Support.
Assisting clients in developing and implementing aftercare plans tailored to their specific needs and circumstances.
Providing educational resources and guidance on coping strategies, relapse prevention, and accessing community support services.
Main Tasks: -
Engage with clients in a compassionate and non-judgmental manner to build rapport and trust.
Monitor client progress through regular meetings, phone calls, or home visits as necessary.
Coordinate referrals to external services such as housing support, employment assistance, or further counselling.
Maintain accurate records of client interactions and progress towards aftercare goals. - Participate in team meetings and training sessions to enhance knowledge of best practices in Aftercare Support.

Desired Skills and Experience
- Experience in providing culturally sensitive support services to Aboriginal and Torres Strait Islander people
- Demonstrated understanding of social and emotional well-being issues within the Indigenous community
- Ability to work effectively within a multidisciplinary team in a cross-cultural setting
- Excellent communication skills, including active listening and empathy
- Proven experience in conducting assessments and developing care plans
- Knowledge of community resources and support networks available to Indigenous communities
- Strong organisational skills and ability to manage time efficiently
- Understanding of trauma-informed care principles and practices
- Understanding of suicide prevention care in the Aboriginal Community
- Relevant qualifications in social work, psychology, counselling, or a related field
- Current driver's license and willingness to travel within the South Hedland region as required
