The Support Worker is responsible for support services aimed at maximising the living, social and recreational skills of people living with a disability and those who are frail aged. Community inclusion, independence, decision making and personal choice should be promoted through these supports, with a focus on individual needs and goals
Responsibilities:
- Supporting clients with personal care, household tasks, shopping, and mealtime support
- Providing emotional support to clients and their families
- Providing active support for clients assisting them to progress towards their goals and develop their general living skills
- Supporting and monitoring the health care needs of clients, including assisting with medication and temperature checks
- Assisting clients with transport to/from appointments and/or social activities or Supporting clients to access the community and recreation programs
- Teaching clients life skills and providing clients with general supervision and general administration tasks
- Understanding the client’s communication needs and adapting your communication to each individual, e.g. sounds, tones, body language
- Portuguese speaker is highly desirable
