- Between $48,558 and $52,999 per annum (pro-rata), plus superannuation
Lakes Entrance Aboriginal Health Association (LEAHA) in partnership with Gippsland Lakes Complete Health (GLCH) aims to improve the health and wellbeing of Aboriginal community members in and around the townships of Lakes Entrance, Metung and Swan Reach.
This role has the primary responsibility of connecting with local Elders, older Aboriginal and Torres Strait Islander people and their families to raise awareness of the Aged Care service system and how to navigate it.
The overarching function of the role is to ensure that support for Elders is organised, sequenced, and supported to enable effective access to appropriate My Aged Care services both internal and external to the organisation.
You will work closely with the Aged Care Support Coordinator. They will also help determine the level of local demand for Aged Care services, identify existing Aged Care services, any service gaps in the area and continue advocacy work with Elders as required.
Qualifications and Experience
No specific experience is required for this role. Willingness to undertake further study relevant to the role would be well regarded.
You will need to be able to develop strong relationships with the local Aboriginal and Torres Strait Islander community.
