An exciting opportunity to join the Ageing Well Services team is now available. You will be positive and passionate about working with residents over 55 years old and their networks to have the best possible experience of Ageing Well in Ballarat.
As a Community Connector you will have experience in providing outreach, short-term case management, and stakeholder engagement. The Community Connector role will provide you with the opportunity to use your knowledge and passion to assist older people, their families and representatives navigate My Aged Care and funded supports, community resources and social linkages to encourage increased well-being and independence for our over 55 residents.
The role sits within a collaborative team who are committed to implementing the Ageing Well in Ballarat Strategy and creating an age friendly community where people feel valued, connected, included and respected and feel confident to participate fully however they choose.
Key Selection Criteria:
- Tertiary qualification in Allied Health or related discipline and/or relevant substantial experience in Assessment, Case Management/Case Coordination or the Community Services sector.
- Proven ability to communicate and work with a diverse range of people experiencing vulnerabilities, including but not limited to Elder abuse, mental health, social isolation, cognitive decline and financial disadvantage
- Demonstrated understanding of Microsoft Office suite and literacy in utilising other digital channels and platforms for engagement and client management purposes
- An understanding of the Aged Care system, reforms and the opportunities and challenges faced by people as they age
- Experience in collaborating with external agencies and organisations to support Ballarat’s Over 55 community
- Experience in coaching and capacity building individuals/groups in their understanding on how to access support services including but not limited to My Aged Care
