GEM@Home is a dynamic and innovative team providing specialised care to older individuals with complex medical, functional, and psychosocial needs in the comfort of their own homes. As a member of our team, you will be part of a collaborative effort led by geriatricians, focusing on multidisciplinary assessment, restorative interventions, and sustainable care planning.
About the Role:
Your responsibilities will include, but are not limited to:
- Providing specific therapeutic interventions under the direction of AHP staff;
- Assisting with patient transport to/from AHP therapy/intervention;
- Assisting patients in preparation for therapy and post-intervention;
- Applying screening assessments, treatments, and utilising standardised assessment tools under AHP staff direction
To be successful in this role, you will need:
- To hold or be undertaking a Certificate 3 in Allied Health Assistance or equivalent,
- Relevant experience in providing therapeutic interventions under the direction of healthcare professionals,
- Ability to work collaboratively within a multidisciplinary team
- Ability to work in a physically demanding environment to assist patient therapy and / or handling of patient related equipment and devices.
