An exciting opportunity currently exists for an AHA to join the Cardiopulmonary Rehab Team, specialising in delivering subacute rehabilitation programs for predominantly for people with chronic heart failure, cardiac disease and chronic pulmonary conditions. Health Independence Programs (HIP) – Rehab & Clinics: are a group of services that provide time-limited, goal-orientated care for people who have a disability, are frail, chronically ill or recovering from injury or illness to regain optimal function.These include the Community Rehabilitation Service, Rehab in the Home, Cardiopulmonary Rehabilitation, Specialist Clinics including the Continence; Cognitive, Dementia and Memory Service (CDAMS); and the Falls & Balance Clinic.The Cardiopulmonary Team operates from our Kew and Fairfield sites.Working at St Vincent’s
