About the role
We are seeking a dedicated AOD Care and Recovery Co-ordination Practitioner! You will support and provide a range of interventions including case management to assist in improving health and wellbeing for individuals experiencing Alcohol and Other Drugs related harms.
Responsibilities include
Evidence based interventions are delivered in line with client centred treatment that aligns with AOD training and qualifications
Case management support provided to clients
Individual treatment plans are developed in collaboration with clients
Clients assisted to access support services and establish community supports
Assistance with other AOD services regions (Mallee) may be asked by Team Leader when needed
This is a fixed-term part-time position, contracted until December 2024 (30.4 hours per week), based in Bendigo, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.
What you will have
Relevant tertiary qualification in Social Work or related discipline
Minimum qualification Diploma Community Services
Relevant experience in Social Service environment
AOD Skillset or obtained within the first 12 months of commencement
Completion of Forensic Fundamentals or obtain within first 3 months of employment
Family Violence foundational, information sharing scheme and brief/intermediate risk assessment training within the first 12 months
A Current Victoria Driver's Licence
