Job description

  • Conducting holistic assessments for consumers aged under 65 in their home, to assist with building capacity to live independently.

About the role

In this role, you will be conducting comprehensive, holistic in-home assessments, ensuring that individuals receive tailored support that meets their unique needs. Beyond assessment, you'll play a pivotal role in connecting clients with a network of both funded and non-funded services, fostering a collaborative approach to holistic care.

Additionally, you'll have the opportunity to arrange brokered services, facilitating access to specialised support when necessary. This role is not just about ticking boxes; it's about creating meaningful connections, advocating for those in need, and empowering individuals to thrive within their communities. In addition to a salary equivalent to SCHADS Level 4 and superannuation, you’ll have the ability to increase your take-home pay by accessing up to $15,900 in salary packaging benefits.

There is also the option to increase your annual leave entitlements through our purchased leave program, access to paid parental leave for up to 10 weeks and can receive a 10% discount on the childcare subsidy gap fee when using a Uniting Early Learning service or centre.

About you

You will bring with you:

  • Understanding of the issues related specifically to the caring role and experience in the provision of care to the elderly, younger people and people with disabilities.
  • Knowledge of principles of community inclusion and participation, with experience delivering a response or project to a community need or service gap.
  • Demonstrated experience in performing community and health assessments using a strengths-based approach and developing care plans.
  • Experience working collaboratively with consumers, carers and relevant agencies to achieve consumer goals.
  • Excellent verbal and written communication, strong Microsoft Office and health literacy skills, and a high level of stakeholder engagement, networking, and negotiation skills.
  • Tertiary qualification in Social Sciences, Disability Studies or Allied Health professions and a current Australian driver’s licence.

About the programs

Home and Community Care Program for Younger People (HACC-PYP) is for people aged under 65 and Aboriginal people aged under 50 who need assistance with daily activities. The program has been designed to support people with disabilities, a medical or health condition, and their respective carers to live as independently as possible in their own homes.

The Support for Carers Program (SCP) is funded by the Victorian Government for unpaid carers. An unpaid carer is a person providing care to another person with care needs. A carer may look after a partner, family member, friend or someone else who needs support. Carers can be of any age, from any culture or country, and any socio-economic background.
