The Team:
This fixed-term ADON - Clinical Service Integration role will lead the implementation of the new CHaPS model of care and the extended sustained home visiting program. The successful candidate must have the skills and capability to drive multiple change projects concurrently with strong change management and organisational skills. The role will be progress implementation of a number of key priorities for CHaPS, including in the areas of workforce, governance, model of service delivery, data and reporting. Along with progressing implementation of the extended sustained home visiting program focusing on the first 1000 days, as funded under the Child and Youth Wellbeing Strategy.
The Role:
- Will progress implementation of a number of key priorities for CHaPS, including progression of the extended sustained home visiting program focused on the first 1000 days.
- Work closely with other members of the CHaPS team to coordinate, and evaluate, clinical input into the wide variety of project tasks, from the various technical, operational and policy contributors, to ensure congruency with the overall service approach.
- Provide senior leadership and expert professional advice to the Nursing Director on projects as delegated utilising clinical systems improvement and clinical service models to inform improvements in service planning and design.
- Establish and maintain mutually productive relationships and negotiate high level agreements with both internal and external stakeholders, and facilitate partnerships with key stakeholders across the DoH, advocating for data integration, information and knowledge sharing.
Details of Appointment:
Fixed term, full time, day work, position working 76 hours per fortnight, commencing from early January 2023 for twelve months.

Hobart, Tasmania
Base Salary: $123,735 per annum + 10.5% superannuation
- Salary range is in accordance with Nurses and Midwives (Tasmanian State Service Agreement) 2019
- Post Graduate Allowance of up to 7.5% based on qualification type
- Professional Development Allowance of up to $750 per annum
Successful applicants will be required to meet the essential criteria:
- Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse
- Current Working with Children Registration
- crimes of violence
- sex related offences
- serious drug offences
- crimes involving dishonesty
- Identification check
- Disciplinary action in previous employment check.
