Our Far North Queensland office continues to grow quickly and we are looking for an experienced and dedicated Care Coordinator to support this continued growth in the Tablelands Region. 

Role Overview

The successful candidate will preferably have a strong background in Community Aged/Disability Care. The role itself is responsible for service development, coordination and delivery, including ongoing client assessments and development of care plans that deliver flexible and personalised in-home support. Successful applicants will thrive on working in a close-knit team and have a passion for delivering the highest quality in client care.

This key role will focus on case management, nursing support, customer service, personal care work where required and general administration.

Major duties include:

  • Care Assessments;
  • Government-Compliant Care Planning;
  • Nursing supports; 
  • Building and maintaining customer relationships and;
  • Quality assurance.

Qualifications

  • Bachelor of Nursing with a current AHPRA registration OR
  • Certificate 4 in Aged Care, Disability or Community Services;
  • Driver's License and comprehensively insured dependable private vehicle;
  • Current National Police Clearance; and
  • Current First Aid & CPR.

Preferred Skills;

  • Good understanding of the Government funding environment for aged care (primarily CDC) & NDIS (preferred but not necessary);
  • Good understanding of the hospital discharge environment;
  • Subscribe to a culture of teamwork, caring and quality service;
  • Excellent communication abilities, professionalism, a good reputation & respect for our staff, clients and their families; and
  • It is also essential to have good judgment, quick decision-making, excellent communication, staff management and administrative skills.
