Role Overview:
Become a Care Coordinator at UnitingSA! Cover a Maternity Leave,starting at the end of March and concluding at the end of November.Organise tailored care, schedule services, and ensure compliancewith funding and legal standards. Manage well-being, safety, andindependence using co-designed Care Plans, collaborating with amultidisciplinary team to achieve clinical care and clientoutcomes. Join us in making a difference!
Key Responsibilities:
- Collaborate with consumers to design individualised care plans.
- Review consumer goals and update care plans regularly.
- Ensure compliance with funding and legislation.
- Participate in various meetings, including multidisciplinary teams.
- Monitor and manage consumer budgets.
- Audit quality and systems as directed.
- Respond promptly to consumer inquiries.
- Assist clients in accessing services through My Aged Care.
- Manage complex consumer situations with team support.
- Maintain accurate clinical records and documentation.
Skills & Experience:
- Familiar with CDC/HCP and CHSP programs.
- Proficient in Aged Care Quality Standards.
- Knowledgeable about My Aged Care system.
- Adept at conflict resolution and problem-solving.
- Effective communication with clients, families, and stakeholders.
- Proven ability to work autonomously and in teams.
- Creative problem-solving and evidence-based solutions.
- Worked in fast-paced community roles.
- Conducted individual assessments, referrals, and case management.
- Co-designed Care Plans in compliance with funding guidelines.
