WHO ARE WE?

Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory.

We work to address the social determinants of health including the broader issues that affect our people's health, through evidence-based research and public advocacy. We also work to empower individuals, families, and communities to optimise their health.

ABOUT THE ROLE

The Care Coordinator - SEWB (CC SEWB) will participate in the provision of comprehensive health care to Aboriginal and Torres Strait Islander people of the Greater Darwin Area. In addition, this position will provide a support role to other health practitioners both within the organisation and the community. The SEWB CC is crucial to maintaining cultural integrity and advocates strongly for our patients. The SEWB CC will provide support to DDHS clients with chronic disease, mental health conditions and complex needs and support them in accessing relevant services according to their care plan with the aim of self-management in Primary Health Care (PHC).

WHAT YOU WILL DO

  • Provide day to day health care service activities within the context of relevant SEWB programs and Primary Health Care in a professional, confidential, and culturally safe manner.
  • Ensure coordinated client care by maintaining assessment, care plans, external service linkage and pathology schedules, referring to and working with allied health services, supporting client self-direction in health care plans, and preparing for visiting specialists' clinics.
  • Actively model DDHS Code of Conduct Policy, Privacy and Confidentiality of Health Information Policy.
  • Demonstrate appropriate and professional workplace behaviours, help team members as required and undertake other key responsibilities or activities as reasonably directed.
  • Effectively communicate and maintain a positive work environment with a multidisciplinary team including clients and external service providers to ensure external stakeholder services to clients.
  • Proactively identify and address WH&S hazards, incidents, and injuries.
  • Ensure all documentation, records management and the health information system database are maintained and completed in accordance with established standards, policies, and protocols to ensure continuity of care.
  • Contribute to developing and implementing strategies to meeting goals set in the provision of comprehensive primary healthcare to the Indigenous people of the Greater Darwin Area.

SELECTION CRITERIA

Essential

  • Qualification as a health professional (e.g., Nurse or Aboriginal and Torres Strait Islander Health Practitioner) and current registration with AHPRA
  • Work in partnership with a multidisciplinary team, to provide and promote a culturally safe and responsive client focused comprehensive Primary Health Care (PHC) service including but not limited to general health care assessment and management, Health Checks, cycles of care, health promotion, mental health plans and prevention
  • Broad clinical experience in primary health care and mental health including skills in planning, coordinating and decision making within a clinical/ case management setting.
  • Demonstrated well developed written, verbal and interpersonal skills including with the ability to communicate sensitively in a cross-cultural environment with Aboriginal clients and staff.
  • Ability to work in a multi-disciplinary team maintaining professionalism, confidentiality, and discretion.
  • Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Controlled Organisations and knowledge of current issues affecting the lives of Aboriginal and Torres Strait Islander people.
  • Experience utilising standard treatment manuals in a primary healthcare and or mental health settings.

Desirable

  • Experience working in an Aboriginal Community Controlled Health Organisation.
  • A tertiary qualification in Mental Health Nursing or Psychology along with a current registration with AHPRA.
  • Demonstrated experience in mental health and social and emotional well-being assessment, treatment and rehabilitation methods appropriate for Aboriginal and Torres Strait Islander people.

IN RETURN WE OFFER

  • 6 weeks annual leave.
  • 5 days paid leave during end-of-year shut down (additional to annual leave).
  • Salary packaging up to $15,990 per annum, plus additional packaging options.

EMPLOYMENT IS SUBJECT TO

  • Willing to undergo a Police Check.
  • Ability and willingness to obtain a Working with Children Clearance (Ochre Card).
  • Current About giving vaccines (AGV) certificate or equivalent.
  • Current driver's licence.
  • Current CPR certificate or the preparedness to gain one within 6 months of commencement.
  • Fully vaccinated against COVID-19 or an exemption as listed on the Australian Immunisation Register (AIR) be submitted.
  • Full rights to work in Australia.
