Key Responsibilities include but are not limited to:
- Contribute to the development, implementation of BSP, monitoring and review of care plans to meet participant’s NDIS goals and to promote quality of life.
- Liaising with NDIS support Coordinators and maintaining good relationships with stakeholders
- Preparing fortnightly or monthly reports related to the progress of the participant’s needs.
- Ensuring that the staff deliver high-quality and personalized care to participants.
- Ensuring health and safety compliance for participants and staff
- Ensuring that all activities such as personal care and medication administration meet the required standards.
- Participating in Monthly On-call roster and support staff after hours
Qualifications and Certifications:
- A Tertiary qualification in Community Services, Mental Health, Disability, or other related Allied Health discipline.
- Current First Aid certificate
- A current full NT Driver’s License and a reliable motor vehicle
- A current NT Ochre Card
- An NT Police Check
- NDIS Worker Orientation Module
Experience:
- A minimum of 2 years of experience working as a Care Coordinator or within the disability, and mental health sector.
- Have experience in the disability field and demonstrated solid knowledge and understanding of the NDIS including mandatory reporting, Quality and Safeguards, NDIS Price Guide, and flexibility within budgets.
- Demonstrated skills and knowledge in the area of planning, implementing, reviewing, and evaluating effective service plans.
- Demonstrated ability to work in a client-focused environment to build client capacity and provide choice and control to manage participants’ service plans.
- Demonstrated ability to case manage several complex cases concurrently, with an emphasis on participant life goals.
- Excellent attention to detail, problem-solving, organisational, and planning skills.
- Ability to support a broad range of participants with varying needs and work independently with minimal supervision.
- Demonstrated ability to use computer IT systems including Client Management software, PRODA.
