The District Nurses (TDN) has been delivering in-home community health care to Tasmanians for over 125 years maintaining a primary healthcare focus in all our services. TDN is currently merging with South Eastern Community Care, presenting an exciting opportunity to enhance service capacity and delivery to our valued clients.
We are seeking an exceptional Care Coordinator who has a passion for helping others, a strong commitment to client care, and the ability to work collaboratively with a team of healthcare professionals to deliver the best possible outcomes.
To excel in this role, you must be an excellent team player with strong communication and organisational skills. Ideally, you will have a sound understanding of the Commonwealth Home Care Support and HACC programs and be able to assist clients to identify and access services to maximise their independence and maintain their wellbeing.
Your responsibilities as a Care Coordinator will include:
- Conducting assessments: including identifying any health risks, medical conditions, or social and environmental factors
- Developing care plans
- Coordinating care
- Providing support and education.
