In this position as a Care Manager with Mobility – part of APM Group of Companies, you will be responsible for providing essential support, advice, and guidance to care recipients, their representatives, and the care and support team.
You will make an impact by:
- Conducting, monitoring, and reviewing validated assessments to inform care planning and review processes.
- Ensuring annual package budgets are utilized in accordance with relevant guidelines and principles.
- Developing and reviewing care and associated plans in collaboration with the care team.
- Ensuring compliance with Aged Care Quality Standards, Aged Care Code of Conduct, Aged Care Charter of Rights, and relevant regulations.
- Ensuring effective stakeholder engagement, communication, and referral processes.
- Overseeing timely and compliant care recipient exit processes.
- Investigating and addressing care recipient incidents, feedback, hazards, and risks.
- Contributing to continuous improvement initiatives within the team.
To be successful in this role you will have:
- Registered Nurse qualification and Registration
- Current unrestricted General Registration with AHPRA (for Nurses).
- Demonstrated supervision, development, and coaching experience.
- Strong understanding of Australian Aged Care Quality Standards and the Home Care Program.
- Excellent communication and stakeholder management skills.
