Job description

As an experienced and passionate Care Manager, you will lead a team of professional and committed staff to provide quality care and support to the residents of Drayton House, a 62 bed building, part of Beresford Hall.

You lead by example and understand the intricacies of working in a residential Aged Care environment from both a staff and management perspective which enables you the ability to make informed decisions.

Your knowledge of the current Quality Standards and the imminent changes as the sector moves to the New Strengthen Standards together with your experience in a role with responsibilities around Continuous Improvement and Clinical Governance will set you apart from other applicants.

Reporting to the Facility Manager, you will be supported by a team of experienced RNs as well as an experienced and supportive off-site Clinical Quality and Compliance Manager and Group Operations and Risk Manager.

You will be required to oversee the clinical service delivery of Drayton House by maintaining accreditation standards and clinical governance and leadership, improving clinical standards, support and mentor staff and promote a positive workplace culture.

Day to day responsibilities

  • Co-ordinate the clinical and health care needs for all residents
  • Competency assessments on staff
  • Undertake resident/family case conference with applicable staff including GP and/or allied health team as required
  • Reviewing of Care Plans
  • Assist with the development and maintenance of clinical documentation
  • Oversee the Medication Management Policy
  • Multi-disciplinary team meetings
  • Provide support to the Infection Control Coordinator and oversee management of Infection Control Program.
  • Monitor ANACC classifications and place requests for reassessment as required. 
  • Monitors the safety of staff, residents and visitors and initiates action to eliminate/reduce the associated identified risks.
  • Coordinates professional care practice including assessment, care planning, the delivery of resident care according to individual care plans and documented policies and procedures, and the review/evaluation of the effectiveness of strategies/interventions, in partnership with residents/representatives.  
  • In consultation with the Facility Manager assist the Roster Coordinator with recruitment, selection and appointment of care staff.
  • Be aware of the rights and responsibilities of the residents and follow up concerns raised by residents or their families.
  • Plan, allocate and delegate duties to staff according to their experience, competence and scope of practice.
  • Safe medication management processes including supervision of staff administering medications and management of dangerous drugs including S8 checks.
  • Participate in quality improvement activities including the follow up of incidents and completion of audits.   

 In order to be successful in this role, you must have:

  • Degree in Nursing & current APHRA Registration
  • Exceptional clinical skills & experience
  • Minimum 3 years Aged Care clinical management
  • Significant knowledge of the Accreditation process
  • Understanding of the Aged Care Quality Standards
  • Understanding of the reporting requirements for SIRS (serious incident response scheme)
  • Working knowledge of ANACC processes
  • Be able to work independently and also be able to work in a team
  • Excellent computer knowledge
  • Current Police Check / NDIS Worker Screening Clearance
  • Have a positive outlook and passion for Aged Care and have a caring, compassionate and honest disposition.
