Care to Care is a registered NDIS, approved HCP and DVA provider, delivering in-home assistance to Aged and Disabled clients. We enable our clients to live their lives independently in their own homes and local communities and pride ourselves on our personalised service delivery. We operate across Brisbane and our main office is located in Cannon Hill.
Main Responsibilities:
- Provide clinical leadership and input to the Management Committee
- Leading a team of 6 to 8 Care Coordinators (EENs and RNs) on a day to day basis
- Clinical guidance for Care Coordinators and Support Workers
- Direct and indirect delivery of clinical care, including medication management, complex wound management etc.
- Client onboarding (assistance with MAC process)
- Client assessments, Care Plan development, Support Worker matching & scheduling
- On-call, shared across the team, on a rotational roster
- Compliance, incident (ie SIRS) and feedback management
- Ongoing Continuous Improvement and Risk Management
- Administrative activities, using the Care Management System
Key requirements:
- Current AHPRA registration with no restrictions - Registered/Clinical Nurse
- Willing and able to act as a Key Personnel
- Strong clinical knowledge – minimum of 5 years Community Nursing/Aged Care experience
- Detailed understanding of HCP and DVA Community Nursing preferred
- Experience with NDIS would be highly regarded
- Strong leadership skills – a natural leader with previous experience in leading/transforming a team or mentoring/coaching would be ideal
- Must be a team player, supportive and personable – a strong personality who can make decisions and give directions with a smile
- Able to mentor the Care Coordinators, look at succession planning, opportunities to learn and coach them to improve their skills
- A good listener with a positive, warm & friendly personality
- Calm natured, high on emotional intelligence and mentally resilient

- Current Criminal History Check clearance
