We now have an exciting opportunity for an experienced, enthusiastic and caring person who would share this commitment and join our team as a Home Care Manager to provide support and guidance to our clients living in the community so they can maintain their independence and quality of life
About The Role
As a Home Care Manager you would be an actively engaged member of the CPE Group Client Management Team, planning, implementing and managing home care services for CPE Group’s clients. The role involves answering initial enquiries, developing relationships with potential and existing home care clients and their families and working with the clients to develop a care plan that is unique to each one. This is a permanent full-time position.
The role supports a balance of office administration days and travel days and will require you to undertake home visits to conduct initial assessments, client sign ups and to identify and develop with the client, appropriate strategies and services which meet each client’s needs and expectations and to carry out ongoing client reviews to ensure their wellbeing.
You will work closely with the Procurement Manager, utilising CPE Group’s centralised procurement systems for referrals and purchases.
You will be reporting directly to the Care Manager - Team Leader and be responsible for the day-to-day management of client care to ensure the delivery of the highest level of client focused home care services in accordance with our Clinical Governance Framework, policies and procedures.
You will oversee a portfolio of support staff, providing mentorship and guidance in the delivery of care to ensure client’s receive high quality support services.

We Are Looking For Someone Who

- Will ensure services meet client expectations and are delivered safely for both clients and staff
- Is able to communicate effectively with clients, their families and carers as well as other health care professionals involved in the client’s care
- Operates in accordance with all relevant regulatory requirements
Essential Criteria:
- Previous experience as a Home Care Manager and knowledge of relevant government funded programs
- An understanding of regulatory compliance requirements and the Aged Care and NDIS Quality Standards
- Excellent verbal and written communication skills including conferencing with families and carers
- Strong computer skills and experience with client database systems
- Excellent care planning capabilities
- An understanding of professional boundaries, privacy, and confidentiality
- Current National Police Clearance
- Full COVID and Flu Vaccinations
- Current Australian Drivers Licence and reliable car
