Code Black Psychology is a specialised multidisciplinary organisation consisting of psychologists, social workers, allied health professionals, and legal practitioners. Through teamwork we provide complex case consultations, diagnostic reviews, care team support and coordination, behaviour support and change, NDIS specialist support coordination, bespoke training and advice on policy and framework development.
Our Care Plan Coordinators provide services to care teams supporting persons with complex needs who are persistently at risk of harming themselves and others. We undertake care coordination within the parameters of the Multiple and Complex Needs Initiative, as well as potentially some specialist support coordination for the NDIS. Care Plan Coordinators do not work directly on a therapeutic basis with the complex client: rather they coordinate and support the services working with and for the client. Care Plan Coordination is a chance to develop and demonstrate your leadership skills as you coordinate and support care teams in their work.
A client with complex needs may present with one or more of:
- High-risk, high-harm behaviours and involvement with the criminal justice system;
- Multiple and dual diagnoses with complex healthcare and support needs;
- Significant health, welfare and other community-based service needs characterised by challenges with engagement and limited participation with supports provided;
- Multiple vulnerabilities such as having more than one disability, having a history of multiple hospitalisations and incarcerations; and
- Disconnection from culture, community and identity.
The successful applicant will demonstrate familiarity with this group through significant relevant previous experience, together with strong organisational, administrative and stakeholder management skills. You will lead the development of a targeted support response for and with the client which requires shared problem-solving, decision-making, planning and independent expert input.
To thrive in this role you should be organised, proactive and a confident communicator. You enjoy working with stakeholders, implementing services to improve the wellbeing of clients, monitoring outcomes and reporting progress. You are curious, a keen learner, a flexible thinker and open to new ideas. Cultural awareness and inclusivity are key elements of your practice.
Your interests and experience will help shape the role, particularly where your skill set can enhance the team’s ability to provide clear case formulations, diagnostic clarity, and agile interventions. Our team members are rewarded with above award remuneration together with development and career enhancement through regular supervision, formal structured and unstructured professional development opportunities, and an organisational focus on continuous learning.
