What You’ll Be Working On
In this role you will manage the My Aged Care portal including incoming referrals, reassessment requests and updating client information and services.
Your day to day involves conducting community care assessments and providing direct client support in consultation with relevant service Coordinators.
You will also provide direct client support in consultation with relevant service Coordinators.
What You’ll Bring to the Role
To be successful in this role you will have tertiary qualifications in Social Work, Psychology, Community Services or other relevant qualification.
You will have demonstrated experience providing assessment, information, referral support service for older people and/or people with a disability and their carers. Experience working with people with dementia would be an advantage.
In addition to the above, you will also hold a valid First Aid Certificate or willingness to obtain, a valid C Class Drivers licence and willingness to drive and use Council vehicles.
This position is subject to a National Police check conducted by Council.
