About the Position
As a Case Coordinator, you will facilitate support and resources for clients through teams providing direct services to clients, along with managing a case load and/or chairing Care Teams. This will involve oversight and collaboration with various service providers, agencies, and professional to ensure a seamless delivery of services. Case coordinators may handle logistical aspects of service provision, such as scheduling appointments, arranging transportation, and administration. You will also enable communication among different stakeholders involved in client support, promoting a collaborative and integrated approach. Case coordinators are responsible for maintaining accurate and up-to-date records of client interactions and service provision. This position is a fulltime role, located within our housing and homelessness service QHome.

- Undertake specialist work in a particular area, requiring expertise and advanced knowledge.
- Develop and implement service delivery programs and work plans, in accordance with organizational objectives and regulatory requirements.
- Provide high level advice to senior management, staff and external stakeholders on complex issues and decisions.
- Manage and oversee the work of a team, including recruitment, supervision, coaching, and performance management.

Key Relationships
- Works collaboratively with Anglicare NQ Executives, Managers, Staff and Volunteers.
- Develops and maintains positive relationships with Anglicare NQ partners, other Non-Government Organisations (NGO) and local community businesses.
- QLD and Commonwealth Government Departments, e.g. Department of Human Services, Department of Children, Youth Justice and Multicultural Affairs and other government departments as required.
Special Conditions
- Hold a current QLD drivers licence ‘Class C’ or interstate equivalent
- Blue Card (Working with children card)
- Satisfactory Employment Screening Assessment(s) required for this role in line with Anglicare NQ Policy:
- National Police Check - Out of hours may be required from time to time
- Client transportation required and/or client visitation outside of the office required
- Valid First Aid & CPR certification
Selection Criteria
Essential
- Knowledge of organisational programs, policies and operations
- Sound discipline knowledge with experience, training and education
- A Degree with relevant experience, along with qualifications in more than one discipline
- Lesser formal qualifications may be acceptable if an individual has specialised skills to perform at this level
Highly Regarded
- Experience working with Aboriginal and Torres Strait Islander people, families and communities.
- Experience working with people from culturally and linguistically diverse (CALD) backgrounds.
Competencies
Team Leader
An individual at the Team Leader level is a front line Manager or Supervisor who has a team of people reporting to them. The team generally has a single disciplinary focus. An individual at the Team Leader level typically reports to a Manager.
