Your Opportunity Reconnect is a community program providing support to at-risk youths aged between 12-18 in the Peel Region. We aim to prevent homelessness through voluntary/self-referrals, emphasise family reconciliation and community engagement and support with accommodation, counselling, mediation, providing person-centered support. Join us in a vital role supporting youth and families through integrated case management. Provide impactful informal counselling, advocacy, and personalised support to prevent youth homelessness. Your key responsibilities will be to: Respond to referrals of individuals and or families from internal and external services including young people, schools, families, other services and conduct initial assessments of suitability for supportUndertake formal face-to-face assessments of clients, including all necessary paperwork using the approved assessment toolsInduct clients into the service including the program requirements, their rights and responsibilitiesDemonstrate knowledge of the National Principles for Child Safe OrganisationsProvide intensive ongoing case management sessions (formal and informal) with clients and review progression against individual support plans and provide informal counsellingLink clients to relevant support services, which will assist in addressing issues including, health, education, employment, recreation and social networksAssist clients to source and secure long-term stable accommodation if required Requirements for Success A Degree or Diploma in Social Welfare/Social Science or a minimum of six years' experience in a similar roleA comprehensive understanding of issues impacting vulnerable and disadvantaged young peopleExperience working with parents of disengaged young people and their familiesSound knowledge of and relationship with referral sources, government and non-government agencies Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
