The Role
We’re looking for someone who is passionate about mental health to fill the vacant position of Case Manager. The position sits within our headspace Early Psychosis (hEP) Mobile Assessment Treatment Team based out of our Joondalup Office. As part of the multidisciplinary team, the Case Manager will provide comprehensive mental health assessment and acute home-based intervention to young people who are newly referred to the service, as well as the provision of crisis intervention and community treatment to current young people treated within the service.
Case Managers will be required to work on a rotating roster across extended hours including evenings, weekends, and public holidays, and participate in on-call system.
The successful candidate will
- Provide case management within a community based multidisciplinary team (allied health, nursing, and psychiatry).
- Assess and treat 12–25-year-olds experiencing moderate to severe symptoms of psychosis.
- Provide clinical interventions with a focus on “least restrictive practice” and upholding principles of the Mental Health Act 2014 (WA).
- Ensure seamless care delivery, inclusive of families, from triage to assessment – referral sources inclusive of families, other community mental health services, community-based psychologists, schools, and inpatient settings.
- Provide care coordination (psychosocial, psychiatry appointments, metabolic and other physical health screening), individualised clinical treatments and family support.
- Provide crisis interventions and manage risk within established clinical framework.
- Possess the relevant Tertiary qualification (Registered Nurse, Occupational Therapist, Social Worker, Registered Psychologist) and hold membership to the appropriate professional body (AHPRA or AASW).
