As a Case Manager, you will play a crucial role in building strong relationships with clients, stakeholders, and team members, while supporting clients with their individual needs and goals. In addition you should ideally have worked with local Aboriginal and Torres Straight Islander Peoples both Elders and People with Disability. A strong knowledge of the NDIS systems would be considered an advantage.
You will be responsible for ensuring quality Care and support to enable more older people and people living with a disability to live stronger lives. You will be responsible for the planning and delivery of timely Care and support that meets participants' goals.
You will have the chance to showcase your case management skills and develop consumer-directed care plans, ensuring alignment with company values and policies.
With your communication and negotiation skills, advanced planning, organisational, and time management skills, you'll be able to respond to client feedback and manage client issues and outcomes with ease.
You will also be responsible for scheduling staff to meet your clients' needs.
You will have an excellent focus on compliance and documentation, understanding the regulatory responsibilities of the Aged Care and NDIS systems.
