About the Opportunity
We currently operate six houses, offering a safe and nurturing environment for our eleven children and young people who require out of home care. You will be directly responsible for four residences and mentor our care coordinators, who are responsible for one residence each.
Although there is a lot more to this role, some key responsibilities include:
- Conducting assessments of clients' needs, strengths, and well-being and producing reports, plans and household documents.
- Coordinating and monitoring the implementation of case plans in consultation with Child Safety and other stakeholders.
- Establishing a therapeutic and homely environment that positively manages behaviours, by constantly monitoring and implementing safety standards and protocols for each placement.
- Working toward permanency goals for each client, while planning and coordinating safe transitions as they occur.
- Driving team excellence through team selection, rostering and performance monitoring for each placement
- Being a leader by inspiring, guiding and supervising the senior support workers across all placements so our teams provide high-quality support and care that leads to a positive outcome for each child.
A key part of this role is providing on-call support and professional advice when crises happen in placements. This requires a willingness to operate and take shifts in the out of hours on-call roster.
This role takes commitment, empathy and organizational prowess. Every day involves juggling care plans, liaising with a lot of stakeholders, solving problems and sprinkling positivity around meeting the needs of our incredible kids.
What you bring
- Qualifications: A degree in Social Work and/or relevant qualification a related field.
- Experience: Demonstrated and proven experience in case management or similar role, preferably within disability support or out-of-home care settings.
- Sector Knowledge: A deep understanding of best practices, regulations, and trends within the out of home care industry.
- Leadership Skills: Strong leadership abilities, with the capacity to inspire and motivate a diverse team to achieve common goals.
- Communication: Excellent verbal and written communication skills, allowing you to effectively collaborate with internal and external stakeholders.
- Organizational Skills: Exceptional organizational abilities, including the ability to prioritize tasks, manage resources effectively, and meet deadlines.
You will also need:
- Positive Notice Working with Children (blue card), or exemption,
- NDIS Worker Screening Clearance (yellow card), or exemption,
- LCS or the ability to obtain prior to commencing,
- Clean and current Queensland driver licence
