ABOUT TOWNSVILLE MULTICULTURAL SUPPORT GROUP (TMSG)
Townsville Multicultural Support Group (TMSG) is a community organisation assisting people from culturally and linguistically diverse backgrounds to achieve independence and social inclusion for over 30 years in regional Queensland.
TMSG, HEAD TO HEALTH ADULT MENTAL HEALTH HUB AND NEAMI NATIONAL CONSORTIA
The Case Manager with Townsville Multicultural Support Group (TMSG) works in collaboration with the Head to Health Adult Mental Health Hub and NEAMI National consortia. This position will have reporting obligations to Head to Health Adult Mental Health Hub management via the TMSG Operations Manager and CEO related to the delivery of the Adult Mental Health Hub contractual obligations aligned with case management support services.
The Case Manager position aims to work with the local CALD communities to identify and support people living with high levels of stress, moderate, severe, complex and trauma related mental health conditions with access to timely mental health support services. This position will work with the local CALD communities to develop comprehensive case management plans to assist in timely service supports. This includes access to culturally sensitive referrals, appropriate group healing services, and service navigation to external local networks. Case management aims to build the knowledge and skills to increase mental health care to enable greater social and economic participation and contribution to the local community. The Case Management position reports to the TMSG Operations Manager to ensure a wholistic approach to service support access.
KNOWLEDGE AND EXPERIENCE REQUIREMENTS
  • Demonstrated high level experience working with individuals and families from local CALD communities from humanitarian backgrounds and local related networks and services.
  • Demonstrated ability to understand the barriers and systems that impact on people in local CALD communities living with high levels of stress, moderate, severe, complex and trauma related mental health conditions.
  • Demonstrated ability to assess local CALD Community members needs to document and develop a succinct, clear case management plan that includes referrals to Adult Mental Health Hub and/or to external networks.
  • Demonstrated ability to work with local CALD people living high levels of stress, moderate, severe, complex and trauma related mental health conditions.
  • Demonstrated ability to manage high expectations of people seeking mental health related support.
  • High level written and verbal English communication skills.
  • The ability to form and maintain effective professional relationships/networks with external service providers.
  • Demonstrated capacity to work according to contractual obligations as outlined in the Head to Health Adult Mental Health Hub Adult Mental Health Hub’s subcontract.
  • Computer literacy, including proficiency in the Microsoft suite of products.
DESIRED
  • A tertiary degree in human services, social work, or psychology.
  • Ability to communicate in another language other than English.

TASKS WILL INCLUDE, BUT ARE NOT LIMITED TO:
  • To ensure clients are fully assessed of their needs and are delivered mental health services in accordance with the Adult Mental Health Hub’s framework, as well as referred to other TMSG and external services to meet client needs that may fall outside of Adult Mental Health Hub’s contractual requirements.
  • Work collaboratively with the nominated Head to Health Adult Mental Health Hub personnel.
  • Provide comprehensive mental health assessment, case coordination, intervention or group work as required.
  • Participate in the identification, development and implementation of quality and service improvement activities that continually improve client care and enhance the delivery of services.
  • Conduct culturally sensitive and appropriate group healing sessions to meet local needs and demand.
  • To ensure all services are provided in an efficient and culturally sensitive and appropriate manner to meet Key Performance Indicators as required under the Adult Mental Health Hub subcontract.
  • To maintain specified administrative, procedural, compliance, reporting, and communication requirements as stated in the Adult Mental Health Hub subcontract with TMSG.
  • To maintain communication with the TMSG Operations Manager on the progress of people receiving case management services, including any identified emerging needs.
  • Assist the Community Engagement position in developing engagement, awareness, and educational strategies to ensure continual access and service improvement.
  • Willingness to be available for a rostered after-hours phone contact according to TMSG and Head to Health Adult Mental Health Hub policies and procedures.
