Located at Intercept in Caboolture, Compass is an early intervention and prevention program providing support to young people between the ages of 10 and 16 who are at risk of entering the Youth Justice system and their families with targeted and holistic complex case coordination and intensive case management through stakeholder engagement, counselling, group work, mediation and practical support to the whole family.
About the role:
As a Senior Case Manager you will deliver early intervention and prevention services for young people aged 10 to 16 years who are at risk of entering Youth Justice, and their families. Responsibilities will include:
- Undertake initial and ongoing assessment of client needs, including the establishment of goals for the intervention period;
- Undertake case management, advocacy and mediation support for young people and their families
- Provide sustainable interventions for clients and their families ensuring that all casework is underpinned by evidence based practice
- Assist in the development and facilitation of targeted individual and group programs for young people and their families/carers;
- Contribute to service planning including the development of work plans, evaluation of services provided and subsequent reporting;
- Participate in networks and forums to promote, enhance and develop links within the community
- Work closely with key agents such as child safety, police, health and educational links
This is a part-time opportunity (76 hrs fortnight) and will end on 30/06/2025.
About you:
The successful applicant will have:
- Qualification in social work, social or behavioural sciences, or equivalent experience;
- Strong verbal and written communication skills,
- Experience working within community services,
- Knowledge of statutory requirements, program and organisational policies and procedures.
- This position requires maintenance of a positive Working with Children (Blue Card) check and current Australian Driver’s License.
