Case Worker, you'll be responsible for providing hands-on assistance to individuals in the Community Living Support (CLS) program. Your role involves implementing the core values of Open Minds and using Motivational Interviewing and Acceptance and Commitment models as a framework for service delivery.
You'll collaborate closely with colleagues, team leaders, and other service providers to ensure efficient service provision. Additionally, you'll track and report service outcomes against evaluation criteria and key performance indicators to measure the effectiveness of the support provided. Join us as a CLS
