- Working closely with the State Managers, Coordinators, Assessment and Marketing Teams, to build a pipeline of referrals and new consumer leads to meet shared targets and KPIs.
- Partnering with individuals and their families to help them navigate the aged care system and identify products and services that are most suitable to their needs and wants.
- Communicating empathically with people from various backgrounds
- Building positive relationships with internal stakeholders throughout the wider St Vincent’s Network.
- Collaborate with the wider customer experience and operational teams to assist with regular community and networking events to promote home care, as part of a holistic offering.
- Develop new streamlined education/nurturing, pipeline and admission processes & systems to support the efficiency intake & client conversions
- Maintaining data integrity across several tools, systems and databases to provide accurate and usable consumer data to support
- Travel to locations suitable to the client for sales meetings and presentations
- Maintain an advanced understanding of home care funding & services, care planning, eligibility and access considerations and be able to convey to prospective clients and their families
- Ensure that prospective clients are given the necessary information package and that the information within the package is both current and explained clearly in a professional manner.
- Demonstrate a caring, personalised and individualised approach to all interactions with existing and future customers, their families, support networks, influences and clinical referrers
- Work collaboratively with the team to implement ideas for business development
- Relevant industry experience, especially in care coordination, intake or client relationship roles
- Strong understanding of community based aged care funding, specifically the Home Care Package program
- Prior experience in navigating and managing My Aged Care portals
- Collaborative team work and a commitment to quality improvement initiatives is integral to the role as is working within a proactive learning model to guide professional development opportunities
