POSITION OVERVIEW:

RESPONSIBILITIES:
- Service promotion led to the growth of Choices and Rights, both in reputation and client numbers.
- Strong referral connections with clients, caregivers, or guardians, support coordinators, and other business partnerships while maintaining strong relationships.
- Meet or exceed sales targets by promoting disability support services, securing contracts, and driving revenue growth.
- Prepare regular reports on business development activities, achievements, and challenges for executive review.
- Facilitate the intake and assessment of new clients for the service by liaising with families, NDIS representatives, and allied health professionals to process and receive new referrals.
- Negotiate funding and service arrangements for transitional support in line with the NDIS price guide.
- Ensure adequate information is gathered regarding individuals’ immediate support requirements.
- Develop support plans for each individual once accommodation commences by undertaking a detailed assessment to ensure the “best fit” in the development of support plans to address their personal, health, and skill development needs.
- Adhere to Choices and Rights policies relating to confidentiality and record management.
- Manage all compliance documentation from referral to service delivery.
To be successful in this role, you will have:
- Tertiary qualifications in Social work, Disability, Human Services, Allied Health, Nursing or Community services are desirable.
- Minimum 2-3 years of NDIS experience in a similar role.
- STA, MTA, SIL, and in-home service coordination experience preferred.
- Sound understanding of the NDIS price guide, completing the schedule of support agreements, and tenancy agreements.
- Stakeholder relationship, time management and communication skills.
- Problem-solving and initiative, and the ability to be flexible in your thinking.
- Good listening skills and the ability to build meaningful relationships.
- Expectation management skills for clients and stakeholders.
- High-level report-writing skills and database experience.
- Strong referral and sales knowledge.
- Good local area knowledge.
Mandatory qualifications for this role:
- NDIS worker screening clearance (No Card, No start).
- Current First Aid & CPR Certificate.
- Current National Police Check.
- Current driver’s license.
- Full Comprehensive car insurance (preferred but not essential).
