About the Role
Reporting directly to the Aged Care Manager, this hands-on position will provide operational coordination, direction and leadership of the day-to-day service delivery activities, systems and outputs required to successfully deliver the Home Care Package (HCP) program to consumers. As the Clinical Care subject matter expert within the Aged Care Services Team, this position coordinates various Clinical Care quality, safety, compliance, and risk management activities.
Duties & Responsibilities
- Provide leadership and management to HCP Facilitators to ensure quality outcomes are met
- Lead the coordination of clinical needs assessments of HCP consumers to ensure needs are met and regularly reviewed
- Develop and maintain Clinical Care related policies, procedures, and frameworks as they relate to clinical standards and practice - contributing to quality improvement of systems and processes
- Actively contribute to the clinical governance framework and incident management systems by reviewing existing clinical risks, complaints and feedback, and providing relevant recommendations
- Prepare and maintain relevant reports for Senior Management Team and external funding bodies on a regular basis
- Create opportunities to provide education and training to relevant teams within the organisation
- Liaise with a range of internal and external stakeholders including consumers, families, and service providers
- Demonstrated experience working as a Registered Nurse Level 3 (RN3) under the Nurses Award 2020
- Current AHPRA registration as a Division1 Registered Nurse
- Proven abilities in policy development and business process / clinical practice improvement
- Ideally you will have experience in community nursing or a non-profit setting, although this is not essential - what is more important will be your fit for the values and culture of this organisation

- Salary range between $93,860 - $98,108 plus superannuation and salary packaging benefits
