About this Role

The primary purpose of this role is to oversee the day-to-day operations, ensuring effective and efficient service delivery through role modelling, coaching, and supervision of staff.

The role of the Clinical Care Coordinator is to assist and support our Clinical Care Manager in achieving their vision for excellence in healthcare, ensuring outstanding supports at the participant-provider interface. 

The Clinical Care Coordinator is responsible for providing leadership, support, and guidance to support staff in delivering quality care based on assessed support needs, ensuring alignment with the vision and mission of i-Help Disability Services.

Key Responsibilities:

  • Support in the development of support planning, assessment and interventions address the multi-faceted needs of participants and are appropriately documented and evaluated in support plans. 
  • Assist in the development therapeutic interventions that assist participants towards achievement of personal goals that promote their independence and choice & control. 
  • Participate in the onboarding of participants by assessing their initial needs, ensuring they are well-informed about the services and support available to them and facilitating participants orientations.
  • Act an advisor on request based on clinical matters relating to the participants support needs.
  • Share responsibilities of the “On call” support team.
  • As required to deliver direct and comprehensive nursing care to a specific group of participants with complex nursing care needs.
  • To support in the implementation and evaluation of support staff education and development program that is in alignment with NDIS practice standards.
  • Assist in the development of training plans and materials to deliver clinical education to support staff that is participant specific.
  • Assist the Clinical Lead in initiating and evaluating research related to staff and resource management.
  • Fulfil reporting requirements to internal and external stakeholders within required timeframes
  • Coordinate with the People and Culture Team regarding support staff selection and recruitment.
  • Maintain accurate records of case notes related to participant visits and correspondences, including monthly reports to be reviewed by the Clinical Care Manager.
  • Ensure all client complaints and incidents are investigated and resolved in accordance with I-Help Disability Services' policies and procedures.
  • Identify and manage work health and safety issues in the participants homes
  • Monitor and manage support staff reporting and documentation to ensure alignment with compliance and NDIS practice standards.
  • This position requires regular travel within Victoria and to other states. In this role, you will visit and travel to various locations to support i-HDS operations, attend meetings, conduct inspections, trainings and collaborate with multi-disciplinary teams and clients across different states. 

Key Selection Criteria:

  • Demonstrated knowledge or experience in a care coordination role within the disability/ Aged Care sector.
  • Demonstrated Knowledge or experience in understanding the needs of participants with lived experience in disability, their families and carers and advocates.
  • Demonstrated commitment to excellences in service performance, evidence-based practice and improving outcomes for participants
  • Demonstrated commitment to service performance, evidence-based practice and improving outcomes for participants. 
  • Excellent understanding of relevant legislation and its impact on service delivery and practice including the Disability Act 2006; the National Disability Insurance Scheme and Department of Health and Human Services reporting requirements; statutory regulations; quality and safeguarding standards.

A successful applicant must:

  • be an Australian Citizen, Permanent Resident or hold a valid work permit or visa necessary for the nature of the position.
  • have or be able to obtain all relevant clearances such a NDIS Screening Clearance, Working with Children Check Clearance etc. 
  • Be an APHRA Registered Nurse (Division 1)
