SalaryBayside Mental Health Service are looking for a Clinical Care Coordinator to join their team. This position will support the program delivery across the adult and child and youth mental health service to support the clinical pathways and provide clinical leadership to the non-government organisation providing the Way Back Support Service within the Metro South Addiction and Mental Health Service.

About the Role
The purpose of this role is to deliver specialist discipline specific direct and indirect care as required in the delivery of mental health clinical services coordinating care pathways between Metro South Addiction and Mental Health Services (MSAMHS) and the non-government organisation partner providing the “Way Back Service.”
- Manage an allocated workload to deliver discipline specific within the specialty of mental health, including assessment, intervention and evaluation within a multidisciplinary team in accordance with established clinical & departmental protocols, policies and procedures, utilising evidence-based practice and ethical standards and under clinical practice supervision or professional guidance of senior staff.
- Acts as a clinical reference point with the referring team and responds to every-day clinical demands.
- Lead and collaborate on the development of evidence-based clinical practices, policies and protocols within Addiction and Mental Health Services.
