About the Role
Our Clinical Care Coordinator is responsible for the day to day coordination of high quality Clinical Care and service delivery to our residents. Lead, guide and manage the care team by identifying clear team goals and providing clear direction and support to enhance the care and support teams understanding of their duties and responsibilities.
Our Clinical Care Coordinator will be responsible for (but not limited to):
- Assist the Facility Manager to manage the staffing requirements in line with the agreed budget to ensure efficient and effective levels of resident care.
- In conjunction with the Clinical Nurse, coordinate the implementation of assessments and care planning for all new residents on admission.
- Oversee the daily assessments of all residents and ensure appropriate care planning by Registered Nurses.
- Review and develop residents’ care plans and ensure that individual residents assessed care needs, goals and preferences are accurately identified.
- Monitor documentation for incidents of potential mandatory reporting, and discretionary non-reporting, and ensure correct follow up is achieved and the register is kept up to date.
- Assist with the AN-ACC process and ensure that all documentation to support AN-ACC claims has been completed accurately by staff in a timely manner.
- Adhere to company policies and procedures to ensure compliance with legislative requirements and Australian Aged Care Quality Standards.
About you
The successful applicant will have the following skills and experience:
- Extensive clinical experience within the Aged Care Sector.
- Proven skills in clinical assessments, care planning to accurately reflect assessed care needs of residents.
- Proven leadership skills in the recruitment, supervision and development of staff
- Working understanding of AN-ACC and the claims process
- Experience with the accreditation process undertaken by the Aged Care Quality and Safety Commission and other relevant regulatory bodies.
The successful applicant will be required to have the following:
- Registration with Australian Health Practitioner Regulation Agency (AHPRA) as an RN DIV 1
- Willingness to obtain valid Police Check
- Australian/New Zealand residency or a valid working visa
