Our teams manage the clinical and ongoing care management for recipients eligible for Home Care Packages funded by the Commonwealth government. This includes comprehensive assessment, clinical management and support, coordinating delivery and brokerage of services within the package budget and communicating with family and other service providers as required.
The HCP Case Manager Clinical role will:
- Provide comprehensive client assessment and development of care plans to support our clients at home
- Lead clinical governance and manage the quality management framework within Home Care services to ensure care standards are met
- Provide direct and indirect clinical care to care recipients within scope of practice and ensure clinical needs are maintained
- Provide advanced holistic care skills within a Case Management framework
- Contribute to the ongoing development of the program including participating in development of procedures, planning and reviewing of care recipient services
- Meet on a regular basis with the HCP Team Manager to provide reports on achievement of KPIs and budget
- Provides feedback to HCP Team Manager to identify staff needs and positive achievements and further program development
- Mandatory minimum Nursing bachelors degree qualification and APHRA registration as well as comparable experience in HCP or Aged Care
- Willingness to travel to our Parramatta Office for induction and to attend to meetings/training etc.
- Highly developed assessment skills and the ability to creatively problem solve complex care situations
- Knowledge and experience of community-based care and support services
- An innovative and dynamic approach to the provision of care
- Demonstrated skills in networking, liaison and the development and maintenance of strong links with relevant service providers and users
- Ability to manage a budget effectively, work in a team environment and autonomously when required
- Valid working rights in Australia (Passport, Birth Certificate etc)
- Police Check (or willingness to undertake the check)
- Flu vaccinations or willing to obtain
- Covid vaccination evidence in line with current requirements
- Own vehicle/licence
