About the opportunity:
The Clinical Lead/Nurse is responsible for providing efficient and effective management of all resident care and wellbeing. The Clinical Lead/Manager ensures effective documentation of care and service, meeting legal requirements and maximizing financial outcomes.
As the Clinical Lead/Nurse you will be responsible for, in consultation with the Service Manager, developing, implementing, and evaluating the clinical/care service components of the strategic plan and work around our values and relationship-focused approach to celebrating and supporting our participants.
Responsibilities
- Manage the care need of clients within the SIL and Community services, including any clinical care needs.
- Be prepared to be part of the On-Call roster.
- Conduct various service assessments including clinical nursing assessments in order to, develop and monitor care plans to reflect changing care needs.
- Be able to demonstrate the impact of the services provided on the wellbeing of clients through regular reporting to Management.
- Follow up on clients admitted to hospital and coordinate all discharge documents. Reassess support requirements as needed.
- Effectively manage customer complaints and if required work with various teams to provide a solution.
- Be a positive advocate for CMG

- Excellent clinical documentation practice
- In depth understanding and experience with NDIS

- Display a relationship centred approach to care and supervision
- Great understanding of care planning, goal setting and evaluation documentation
Aged care OR Disability Care experience is essential – whilst you require a relevant qualification in nursing, we are looking for the right people to join us and build personally meaningful relationships with our participants, their families, and your colleagues.
