Job description

An exciting opportunity exists for a Clinical Nurse - Adult Frailty to provide the best care for frail adults 18 years and over presenting to the Gympie Hospital Emergency Department and/or admitted to inpatient wards. The CN is expected to demonstrate an advanced level of autonomous clinical and professional practice and decision making in collaboration with a multidisciplinary team.

The Adult Frailty CN is a frontline role within a multidisciplinary team contributing to the screening, assessment, care coordination and comprehensive planning with the frail adult. The Adult Frailty CN will work collaboratively with the consumer, their caregivers, and the broader healthcare team for this potentially vulnerable group. The CN will work in collaboration with the Adult Frailty CNC and the multidisciplinary team to facilitate the best outcomes for the consumers within the Gympie Hospital. The successful applicant will possess advanced clinical knowledge, problem solving and critical thinking attributes that will assist in the application and evaluation of contemporary nursing practice and theory.
Key responsibilites include:

  • Understand your broad responsibilities for safety and quality in healthcare, actively participate in continuous quality and safety improvement activities within the workplace and notify a relevant clinical or non-clinical responsible officer when concerns exist about workplace or patient safety.
  • Practice in accordance with code of ethics, professional standards and legislation affecting nursing practice by maintaining current knowledge and competence within scope of position.
  • Actively participate in a working environment supporting quality human resource management practices including employment equity, anti-discrimination, workplace health and safety and ethical behaviour.
Remuneration based pro rata on equivalent full time value up to $128 057 p.a., comprising salary rates: $52.86 - $56.60 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 6(1) (temporary part time position available until 30 June 2025, 48 hrs p.f.).
